TRACY WILL BE ON VACATION FROM JULY 23 THRU AUGUST 4, 2020:
If you require refills or anything that will need to be taken care of in that time period please get your request in as soon as possibe so that we can get it taken care of before Tracy's vacation.  Thank you for your cooperation and understanding. 

PLEASE NOTE THE FOLLOWING:

Any correspondence received through the patient portal will be attended to within 3 to 5 business days.  If you require a Rx refill, please make sure to give at least a weeks' notice, as Tracy's schedule is unpredictable and giving her enough notice will her allow her to complete your request in a timely manner.   If you need to reschedule, please give 24 hours notice. Failure to do so will cause you to incur a $135 late cancel or no-show fee. Also, please provide Shari with your preferred day(s) & time(s) so that she can reschedule you.

Any requests submitted over the weekend will be reviewed on Tuesday.   We thank you for your patience and understanding.
